Should you require daycare services for your child starting at Good Shepherd School in August 2023, please fill out the attached form.
Please note that transportation will not be available when you register for daycare services 5 days per week during the same period (for example 5 afternoons in daycare, therefore no bus transportation available for the afternoon period).
For parents sharing custody of their child, both parents must fill out a form for their own needs (ensure to indicate on the form that there is shared custody).
